First, does the damn thing make sense? I used the Book of Enoch as the basis, so there are 200 horny angels on Earth. I limit my story to just a few. Also, I got rid of God, the Devil, Genesis, and replaced them. So, do the replacements work?

Most of it comes with the first draft, and revisions are for nurturing and growing the good that's already there, and trimming the stuff that is dragging it down. Sometimes though, something comes up in revisions where I have to scrape down to the story's bones and build something different from it.

Both, honestly. I only set out to tell the best story I could, and that story just happened to demand more time than book 1. Plus, I had already introduced the world and characters, so with book 2 I wanted to take a more nuanced look at all of it (which naturally takes more time as well lol)
