Someone needs to beat these people over the head with that Margaret Pepe paper. I had the same "but the odds ratio in the top quartile is bigger" reviewer complaint when I published a critique of frailty prediction model studies.

Not attacking Jim so he doesn't need defending :) All tools for guiding medical decision making should ideally be trialed as a part of the existing decision making process - and the more useful they are, the easier this is to do. That's all I'm saying.
