I think, of its Pen S, you’d be justified using Penicillin IV. SNAP protocol. In reality it’s much more common to use Cefazolin so I’d probably go with that. Rx as disseminated S.aureus infection with BSI; doesn’t sound like you’ll get source control so 4-6 weeks. IVOS later.
