The "random (super buff) dude who doesn't matter" has obvious reasons to exist -- self insertion, generic appeal, and of course to keep focus on a single character. But honestly, I wish it were used less, especially when actual characters with personalities bring so much more context to a scene.
Show PostSo, especially when porn focuses on a single character, it's pretty common you'll see stuff like this. I do it all the time, too. The faceless crowd with no distinct identity is more a sexual prop than a group of characters, but it definitely has its uses, and I'm not strictly advocating against it.
