However, in my case, I worked weekends as a ward clerk during COVID and still didn't have access because it was an extraordinary arrangement which meant I had to have a clinical person look stuff up so I could inform relatives properly.

Oh, absolutely in agreement with that. The separate issue re controls as a mechanism for preventing access is complex though. I never had access to patient records because I was in a corporate role & there was literally no reason for me to, but for example ward clerks can be redeployed...
