From what I've seen, if PBMs just paid whatever acquisition rate local pharmacies asked for, employer health plans would be completely broke. PBMs exist to exert actual purchasing power against inflated drug pricing across the board.

I can understand the concern about prescription drug ads, especially when marketing costs get folded into the conversation about prices. I just wouldn’t put the PBMs in the same bucket automatically there are a lot of moving parts between the list price and what a patient actually pays.
