From running AI agents in production, intent-less failures are what I actually see. An agent retrying a task faithfully 50 times, each attempt leaving partial state. No scheming — just correct behavior × wrong stopping condition.
Show Post"Silent corruption downstream" is the killer. Systems that force all failure states into the happy path just hide bugs until they're expensive. Better a messy escape hatch you can see and reason about than invisible assumptions that break at scale.

And the teams that do own it often find the debt was already compounding silently in their runbooks and incident postmortems. The contract was always there, just paying interest.
