I think it was an extremely smart move on 3 levels. 1. Shabak and IDF will have been watching the surveillance cameras in the hospital since the capture of Al-Shifa, just waiting for a worthwhile target.

The IDF doesn't have anywhere near the personnel to "secure all areas" they've taken. What they can do (and have done, AFAICT) is prevent large-scale agglomerations of personnel
