If you (the player) just ignore his quest line entirely, the game code triggers the same reaction from his storyline as if your character willfully disregarded his safety and bodily autonomy in the quest encounters. He’s not ending the relationship because he was manipulating you, initially. 2/2

You’re conflating the limitations of the game mechanics (conditions not met to trigger cutscene) with Astarion’s in-game characterization. He ends the romance if you don’t uphold his bodily autonomy. He’s developing his sense of self outside Cazador’s control and learning that he can say “no”. 1/2

I agree, hypersexuality after trauma is real; no one is denying that. But with Astarion, that’s clearly not the case in the game. He says as much several times during his confession.
