Once saw a similar patient who assisted in the slaughter of a sheep in Russia and scraped his hand on a tooth while removing the tongue (a delicacy, apparently). Biopsy can be diagnostic if eosinophilic inclusions are seen in vacuolated epidermal cells. Looks terrible but self-healing (eventually).

Saw a lot of Q Fever during my deployment to Iraq- essentially no one had close animal exposures and most had not even left the urban core of Baghdad. I think of it as wind & dust storm driven transmission akin to cocci with regional animal husbandry practices as the key risk factor.
