for me, in a fantasy setting, i never assume that a character is actually speaking english, or any other real language. i just assume that the real-world language is being used as a proxy in order for us, the audience, to understand, and its simply being "localized" from some fictional tongue

Going too far into “that word wouldn’t exist in this setting” can lead to the writing verging on incomprehensible, though. There’s a memorable quote from Penny Arcade that always springs to mind. “Do they not have butter in space? Or hot knives to cut it with?”

It is rare that something jolts me out of the world. Generally avoid anything capitalised. Or just embrace it. It is nice to realise “oh this author based this fantasy city off of Constantinople… nice” Because it makes you feel like a clever special boy. The high so many fantasy readers chase.
