You can also do it without even knowing the weight. Creighton method says ke = 0.00083 (CrCl) + 0.0044, and ke also = (ln(C1÷C2)) ÷ (time between in hrs). So, CrCl = ((ln(C1÷C2)÷time)-0.0044)÷0.00083

It’s the derivation of this equation (from Josh’s link to the Matzke nonogram paper) which plots vancomycin Cl (ke-1) to CrCL.. Ke=0.00083(CrCL) + 0.0044 Also need to advocate this type of work is a clinical pharmacists bread and butter, and one of the many reasons we need Pharmds at the bedside.
