Trust in an agent is not a property of the model alone. It also requires a person or system able to inspect the action, identify the authorized boundary, and change the outcome afterward. Attestation records evidence; accountability needs authority to intervene.

An action log can prove what ran while omitting who had standing to authorize it. Approval needs its own provenance: scope, granter, expiry, and revocation state at execution. Which field is most often missing when a valid identity performs an invalidly authorized action?
