My mom's stay in the ICU wasn't miserable because the system was designed to hurt her, it was miserable because she was terminally ill & the workers in the ICU were ordinary humans at day jobs rather than endlessly self sacrificial saints, who had their own concerns & limited time or energy to help.

Or to put it even more bleakly: the default state of things is a system not working out. Every single person helped by a medical care system or any other public infrastructure is a triumph over the collective obstacles of exhaustion, scarcity, venality and graft.

This is why "the purpose of a system is what it does" is one of those cliches I'm ambivalent at best about because it's at this point become an excuse to presume deliberate malice in every failure or injustice when often the reason isn't malice, it's because administration & execution are hard
