Agree v much a human factors / inertia / trust thing! We sometimes get resistance from specific ortho surgeons. That's always hard (& irritating that a surgeon is telling an antibiotic specialist what to do). Hoping this will gradually go away.

We definitely switch to highly BA orals rapidly once sepsis resolved & able to tolerate orals. No specific duration of IV. Our reading of the literature is that oral = IV, but less harm. Usually 8-12w depending on size of abscess etc.

A small abscess, wbc/platelets/crp all more than half way down from peak, could tolerate something like linezolid. This usually happens after at least 2 to 3 weeks on IVs though…

Depends on size of abscess, size of patient, organism. There’s no one size fits all 🫤
