@kglesliephysio.bsky.socialYou can have severe fatigue and cognitive dysfunction from another condition, but that doesn’t mean you have PEM. There’s way more to it and failing to acknowledge that leads to over-simplification. I can’t tell you how discouraging it is to see people get this wrong so publicly as often as they do.
Show PostLike, PEM isn’t a feature of other conditions. It’s something we see in ME. That’s basically it. People may *screen positive for bits and pieces* of PEM in other conditions. People may have ME *and* another condition, but it’s very important we’re clear that PEM isn’t a part of other conditions.
Show Post“Currently, diagnosis of PEM is strictly clinical, wherein patients must fulfill criteria for the DePaul Symptom Questionnaire – Post-Exertional Malaise (DSQ-PEM) questionnaire [39].” Incorrect. Strictly clinical? Sure. DSQ-PEM is used to make the diagnosis? Nope. www.cell.com/trends/endoc...
Show PostThe challenge of Long Covid is to know when to lump and when to split. If you’re going to define the disease, I understand the need for a broadly inclusive definition with subsetting. But the subsets shouldn’t be obligated to explain the whole disease. We should resist the urge to have it both ways.
Show PostYou don’t even need to be a scientist or clinician with some exotic background and training or a prestigious academic or clinical appointment or an elaborate funding history to understand this stuff. You just need to be willing to think it through for more than about a minute. It’s simple logic.
