Disorders of sexual development don't make anyone anything other than male or female, any more than any other developmental disorder. They can cause problems, but that has nothing to do with the notion of gender.

Societies don't consider, but share conventions. People everywhere recognize the reality of two sexes, because that's how we reproduce. The word gender has only recently been used in relation to sexual expression rather than linguistically. Beliefs vary widely but aren't necessarily true.
