I am certain that we could have had robust evidence for how to use non-pharmaceutical interventions at the time scale of RECOVERY. Which gave robust actionable evidence for treatment of severe acute covid in the summer of 2020. Vaccine RCT:s read out in October. RCT:s outran everything else.

It would both have answered a critically important pragmatic question and provided a model for testing face masks for control of infecting others under ideal conditions. (Adherence and burden (as time trade-off in work time) was to be assessed by a random survey of staff members).

We got ethics approval but did not secure funding, and started too late. The vaccine rollout started in care homes around christmas 2020. I still think it would make sense to run it during a flu season, but I'm a consultant not an academic and this was born out of frustration that nothing was done.

The trial was to have been run through the Senior Alert health quality registry infrastructure, with outcomes through the Swedish influenza registry or the local disease protection physician structures. We hoped to randomise ~300 Swedish care homes in the spring of 2021.

I designed a trial randomising care homes to personnel and visitors using, primarily, face masks, visors, or both (since using either was standard by then), as an incomplete factorial design, with primary outcome rate of confirmed infections. It ultimately did not happen but not bcs of feasability.
