  I wish journals & guidelines would stop using the term "step down" for orals. "Down" implies you're consciously accepting inferior treatment. But the evidence shows it's categorically NOT inferior and in fact safer.

  Had a patient just yesterday with non-severe ESBL pyelo that we decided to do PO fos in to prevent an admission. I did say give erta x 1 though just since this is something novel for us to do 🫣
