Ver postThis may well (and should) lead to automation, especially on things like railroads where the environment is comparatively closed and straightforward. If it's safer and you're building systems that prevent human error, it's hard to argue against it.
Ver postThat's not how I would word it. In a safe systems approach, it's never good enough to stop with "human error." What caused the human error? Eg did design or fatigue or training contribute? Can you change it to make sure it doesn't happen again? Can you make it so the error can't happen?
