I'd argue there are sometimes cases where the warning shouldn't be issued and it should just go straight to ban, because with the warning you're training them to move their bullshit back just behind the new line, effectively doing the same thing.

This. Adhering to the letter of the rules without fail is a good way to let people exploit that. Sometimes you just have to make a judgement call. Before things get worse.
